What should I bring to my first visit?

Bring your insurance card, photo identification and a list of all medications, vitamins or herbal supplements you take.

Can I check in online?

We’re here to make your visit as simple and efficient as possible. With mobile check-in, you can securely register from the comfort of your home using your smartphone, tablet or computer. Ask us about mobile registration for a quicker, smoother experience.

What should I do if I can’t reach your office?

Our answering services handles calls from 4 p.m. to 8 a.m. on weekdays and throughout the weekend. For urgent matters that cannot wait until office hours, the service can connect you with the on-call provider.

How can I access my medical records?

Call us at(423) 339-1760 to request a medical release form. A signed, completed release form is required, and a fee may apply.

Do you offer a patient portal?

We’re proud to offer secure online access to your health information, making it easier and more convenient to manage your care.

What insurances do you accept?

We accept most major insurance plans. All insurance copays, coinsurance and deductibles are due at the time of service.

What is your cancellation policy?

If you're unable to keep an appointment, we kindly ask for a cancellation call at least 24 hours in advance. Appointments not canceled within 24 hours may incur a fee.
