How can I contact my primary care provider?

To contact your primary care provider, please contact your primary care office or sign up for our patient portal where you can receive the answers to all of your medical questions.

Will my insurance be accepted?

Though TPMG contracts with various insurance carriers, not all TPMG providers actively participate in our carrier plans. To avoid unexpected or additional out-of-pocket expenses, contact your provider’s office to verify that he/she participates with your insurance plan.

What if I don’t have insurance?

If you do not have insurance, please contact your physician’s office to learn about your payment options.

New patients, and patients who have not been to our practice within the past year, are requested to arrive 15-20 minutes early to complete the registration process.

For your convenience, registration forms can be printed and completed before your appointment by visiting your office’s webpage on our website. Before your initial appointment, be sure to contact your previous primary care physician or specialist to request that your medical records be sent to us.

New Patients:

- All medications you are currently taking (include prescription, over-the-counter, and herbal remedies) - Your current health insurance card - Photo identification - A referral from your primary care physician, if required by your insurance plan - A list of allergies to medications or dyes - Immunization record for children - Payment for any required co-pays, deductibles, or non-covered services
