Do you accept insurance and what are the payment terms?
We accept most major insurance plans. All copays, coinsurance and deductibles are due at the time of service.
Is virtual care available?
Yes. Virtual visits are available to new and existing patients using Zoom; new patients should call to schedule.
What do I need for a virtual visit?
An internet-enabled device (smartphone, tablet or computer) with a camera and speaker. If using cellular data, check with your carrier for potential charges. Depending on your visit, a thermometer, blood pressure monitor or flashlight may be helpful. We recommend choosing a private space with a strong internet connection.
How much does a virtual visit cost; will I have a copay?
Copays depend on your insurance. If you have a copay for in-person visits, it will also apply to virtual visits. You may receive a mailed request for payment.
How can I access my medical records?
Contact your provider’s office to request a medical release form. A signed, completed release form is required, and a fee may apply.
